Topical ocular administration of % dexamethasone ( mg/kg/day) on gestational days 10 to 13 produced embryofetal lethality and a high incidence of cleft palate in mice. A dose of mg/kg/day in the mouse is approximately 3 times an Ozurdex ® injection in humans ( mg dexamethasone) on a mg/m2 basis. In rabbits, topical ocular administration of % dexamethasone throughout organogenesis ( mg/kg/day, on gestational day 6 followed by mg/kg/day on gestational days 7-18) produced intestinal anomalies, intestinal aplasia, gastroschisis and hypoplastic kidneys. A dose of mg/kg/day in the rabbit is approximately 4 times an Ozurdex ® injection in humans ( mg dexamethasone) on a mg/m2 basis.

The vitreous acts as a superb medium for bacteria growth, and, in the past, animal vitreous was used as a culture medium. Bacteria, as foreign objects, incite an inflammatory response. The cascade of inflammatory products occurs resulting in an increase in the blood-ocular barrier breakdown and an increase in inflammatory cell recruitment. The damage to the eye occurs from the breakdown of the inflammatory cells releasing the digestive enzymes as well as the possible toxins produced by the bacteria. Destruction occurs at all tissue levels that are in contact with the inflammatory cells and toxins.

Steroid induced glaucoma may develop after application of steroid preparations applied to the skin of the eyelids. This elevation occurs most frequently with chronic use, such as in patients with atopic dermatitis. Close IOP monitoring of these patients is essential and consideration of a non-steroidal topical medication, such as tacrolimus and pimecrolimus, should be considered as an alternative. Elevation in intraocular pressure has also been noted with application of steroids on skin that was not periocular, either from ocular contamination or systemic absorption. 22 Patients should be advised to wash their hands after applying dermatologic steroids or to use gloves.
